Child and Family Catechesis

For families with children in grades Pre-K-6, including families wishing to have their child receive the sacrament of 1st Holy Communion (usually 2nd grade)

Faith Formation classes are held every school year from September-May.

Access the Faith Formation registration form by clicking on this link: Register Online

Fill out the form, make your payment, and submit the form.

For students who will be making their First Communion, take a photo of your child’s Baptismal certificate and email it to mmoosfaithformation@syrdio.org. If your child was baptized at Mary, Mother of our Savior or Our Lady of the Rosary, you DO NOT need to email his/her Baptismal certificate.
